Family members often live out of town and hire caregivers help through an agency or privately. They need a care manager to check in periodically to make sure the caregiver is complying with the plan of care. Care managers can also determine if the client's care needs are changing and may need increased care. When a client has an emergency, requires hospitalization or an emergency doctor visit, the care manager will arrange everything and either go with them, meet them there, or follow up.
